<p>You can update or correct the Serology Results for the Living Donor or Recipient record as follows:</p>
<ul>
<li>Select the Patient Serology section on the Health Tab and double click on one of the results in the  Result column.</li>
<li>In the Patient Serology Window, select the Result or Type column to be updated and select the appropriate data. Update and save.</li>

<p>Serology Results once entered and saved cannot be deleted but can be updated or corrected.</p>
<p>In case of an error, if an additional set of Serology Results is entered, add a comment in the health tab indicating which of the two sets of Results is the valid entry.</p>

								<content:encoded><![CDATA[<p>From the patient details record, you can add attachments at the bottom of the following tabs: Personal, Contact, Health/Assessment (in case of Deceased Donor), Death, HLA and Registration/Activation.  The attachments are viewable by any user that can access the patient record.  This includes users working on the record or viewing it (for the purpose of supporting a match that has been made involving them).</p>
<p>Select the tab where the attachment needs to be added. Under the Attachment section select the <strong>Add</strong> button. The following fields are required:</p>
<ul>
<li>Filename</li>
<li>Description</li>
<li>Include on Side by Side = defaulted to Yes’,</li>
</ul>
<p>Select the <strong>Browse</strong> button to add the file to be attached. Once the file is selected and the description fields are entered, the <strong>Update</strong> button can be selected.</p>
<p>After selecting the <strong>Update</strong> button, the system navigates back to patient record and displays the attachment as a hyperlink (underlined in blue) in the Attachment section.</p>
<p>Select the <strong>Save</strong> icon to apply the changes.</p>
<h2><strong> How do I replace an Attachment?</strong></h2>
<p>Under the Attachment section of the related Patient Tab, double click on the row that needs to be replaced. Once the attachment window is displayed, select the <strong>Replace </strong>button.</p>
<p>Select the <strong>Browse</strong> button to replace the attachment file. Once the file is selected, the <strong>Update</strong> button can be selected.</p>
<p>After selecting the <strong>Update</strong> button, the system navigates back to patient record and displays the replaced attachment as a hyperlink (underlined in blue) in the Attachment section.</p>
<p>Select the <strong>Save</strong> icon to apply the changes</p>
<h2><strong>How do I delete an Attachment?</strong></h2>
<p>Under the Attachment section of the related Patient Tab, select the <strong>Delete</strong> icon to the right of the attachment. The Attachment Delete window is displayed to enter the reason for deletion. This is a mandatory field.</p>
<p>Once the Reason has been entered, the <strong>Delete</strong> button can be selected. After the <strong>Delete</strong> button is selected, the patient record is displayed and deleted attachment is no longer appears under the Attachment section.</p>
<p>Select the <strong>Save</strong> icon to apply the changes.</p>

<p><strong>Minimum Required Tests</strong></p>
<p>eGFR: Serum creatinine on 2 separate occasions; each reported serum creatinine should also report an eGFR using CKD-EPI with the result expressed as mL/min per 1.73 m2.</p>
<ol type="a">
<li><strong><em>If one or both eGFRs are within the specified range</em></strong> for the potential donor’s age, then either an in-range 24-hour creatinine clearance or measured GFR (nGFR), is required.</li>
<li><strong><em>If both of the eGFRs are out-of-range,</em></strong> then a 24-hour urine to assess creatinine clearance (CrCl) on 2 separate occasions AND measured GFR (nGFR) (such as DTPA, EDTA, iohexol, iothalamate) on at least 1 occasion is required.</li>
</ol>
<p>The Acceptable eGFR ranges are as follows:</p>
<ul>
<li>Living Donor age ≤ 30 then acceptable adjusted is &#8216;eGFR ≥ 90 mL/min/1.73m2&#8217;</li>
<li>Living Donor age ≥ 31 and ≤ 40 then acceptable adjusted is &#8216;eGFR ≥ 85 mL/min/1.73m2&#8217;</li>
<li>Living Donor age ≥ 41 and ≤ 65 then acceptable adjusted is &#8216;eGFR ≥ 80 mL/min/1.73m2&#8217;</li>
<li>Living Donor age &gt; 65 then acceptable adjusted is &#8216;eGFR ≥ 75 mL/min/1.73m2&#8217;</li>
</ul>
<p>Once the eGFR results are entered, the system will indicate if the specified range is ‘Acceptable’ or ‘Unacceptable’ in the wizard.</p>

<p>Please note that CTR will not restrict any donors from being entered into the KPD program but if a donor is deemed unacceptable by these new criteria, the system will issue a warning message indicating that the user has to submit the Living Donor Query form to KPD.</p>
<p>The Living Donor Query Form can be located under the Reference Documents section in the main menu of the application.</p>
<p><strong>What Happens When You Try to Update an Old Record?</strong></p>
<p>CTR will issue a warning – “eGFR is not in acceptable range. You must enter at least one 24-HR Urine Creatinine Clearance test result AND at least one nGFR test result.”, if you try to update any data on the health Tab of the donor record that contains GFR results that do not meet the new requirements as outlined above.</p>
<p><strong>Old Donor records must have: </strong></p>
<ul>
<li>A minimum of 2 results</li>
<li>For every eGFR entry in each result, there must be a corresponding entry in either 24 HR Urine Creatinine Clearance or nGFR – based on the eGFR Acceptable Ranges. (as noted above)</li>
<li>Each result can have the same Test date and value for a specific test type</li>
</ul>
<p>Please see screenshot below where an eGFR value is entered using the same date on each historic GFR entry.</p>

<li>Depending on the range of the eGFR, the user may need to add the nGFR or can update results with the 24-HR Urine Creatinine Clearance ONLY</li>
<li>Please do not create new entries if donor already has a minimum of 2 existing results</li>
<li>Once each of the Old records are updated and saved, you can then add your new data</li>

								<content:encoded><![CDATA[<p>It is important that HSP “Sent for Transplant” is recorded in the CTR as soon as possible. This ensures that the CTR’s general ledger for export thresholds is up to date. When the “Sent for Transplant” is recorded, the export is recorded in the general ledger. Previously, when the transplant was recorded, the import and export were recorded in the general ledger. When the transplant is recorded in CTR 2.0, only the import is recorded in the general ledger. The ODO must record the “Sent for Transplant” in order to register the export in the general ledger.</p>
<p>The General Ledger Report provides for each region, the net balance, exports and imports. The net balance is calculated by subtracting the exports from the imports (net balance=imports-exports). Note that the verification that determines if the donor’s region is at their export limit is done during the allocation process and is based on the donor’s ODO province. The General Ledger Report simply reports on the current net balance for a region by calculating the difference between imports and exports.</p>
<p>In order for the ledger to represent accurate information, it is critical that transplant centres record transplants and that ODOs record the “Sent for Transplant” in the donor record.</p>
<p>Recording that kidneys were sent for transplant can easily be done in the Canadian Transplant Registry.  Follow these steps to record the export:</p>
<p>Note: In order to record a kidney as sent for transplant, you must first have run the allocation and have an accepted offer.</p>
<p>1. Locate and open the donor record</p>
<p>2. Click on the Recovery tab</p>

Note that if you select “No” you will need to specify a reason in the Reason column</p>

<h1>Locked out?</h1>
<p>If you are unable to login and you are receiving an error that your account is locked out, then this may be due to too many (5) invalid login attempts.</p>
<p>Please note: The invalid login counter starts at the first invalid attempt and keeps track until a valid login is entered or you reach the maximum number of attempts.  Invalid logins, therefore, can be attempted at different time/day and are accumulated until a valid entry is made or you are locked-out.</p>
<p>Follow the tip below for unlocking your account.</p>
<h5><strong><i class="fa fa-lightbulb-o"></i> Tip</strong>:</h5>
<p>Click on the <strong>Forgot Password</strong> button on the login screen.  You will then be required to enter your username and the email address associated with your account.  Once this information is entered, click on the <strong>Reset Password</strong> button.  In a few moments, you should see an email with a temporary password… this information will be sent to the email address that is associated to your account – the same email that you used when completing the Forgot Password screen.</p>
<p>The Forgot Password process will unlock your account, but it will also assign a new temporary password.  Once you login with the temporary password, you will be required to select a new password.  Please see the section below, titled <strong><a href="#password-rules">Password Rules</a></strong>, for more information.</p>

<p>If you’re unable to login due to an incorrect username or password, please ensure the username is correct and please remember that all <strong>passwords are case-sensitive</strong>.  The username is not case-sensitive.</p>
<p>Follow the tip below to have your password reset.</p>

<h1>Changing your password</h1>
<p>There are different reasons for changing your password; one may be that the application is forcing you to change your password, and the other may be your choice to change the password.</p>
<p>Below are some details for each reason, as well as, tips and instructions for changing your password.</p>
<h4><strong>The application is forcing me to change my password…</strong></h4>
<p>If the system is forcing you to change your password, then it is because you have had the same password for an extended period.</p>
<p>You will see this happen immediately after logging in.  You will be presented with a change password screen.</p>
<p>In this screen, you must provide your current username and password and then provide your new password.  You will also be required to enter your new password again, to confirm that it matches with the first entry.</p>
<p>Please remember, you must meet the minimum criteria for passwords and you are not permitted to use a password that is the same as any of your previous 5 passwords.</p>
<p>See the <strong><a href="#password-rules">Password Rules</a></strong> section for details on the minimum criteria for a password.</p>
<h4><strong>I want to change my password…</strong></h4>
<p>Changing your password at any time is possible.  First, you must login to the application.  Once logged in, be sure you’re on the main page – the Dashboard.</p>
<p>Near the top of the page you will see a <strong>Welcome [your name]</strong> label, where the <strong><em>[your name]</em></strong> part is your first name and it will be clickable (a hyperlink).</p>
<p>Clicking on your name, which is a hyperlink, will bring you to your user profile and preferences.  Within this screen you will see fields for <strong>Current Password</strong>, <strong>New Password</strong> and <strong>Confirm Password</strong>.  You will need to enter your current password, along with the new password and the new password again in the Confirm Password field.</p>
<p>Enter your current password. As you enter your New Password, the system displays hints in choosing the password.  Once the chosen password meets the prerequisites, the hints are no longer displayed.</p>
<p>Please remember, you must meet the minimum criteria for passwords and you are not permitted to use a password that is the same as any of your previous 5 passwords.</p>

<p>When entering, a new password remember that it must contain:</p>
<ul>
<li>At least one upper case letter</li>
<li>At least one lower case letter</li>
<li>At least one number</li>
<li>At least one special character (~! @#$%^&amp;\*()-_=+[{]}\|;:,&lt;.&gt;/\?)</li>
<li>The password must be at least 8 characters long</li>
<li>Cannot be the same password previously used (5 instances)</li>
