A duplicate record was found in the system, but I don’t have access. What do I do?

Although the Canadian Transplant Registry has several mechanisms in place to prevent duplicate records from being created, it is always possible that some duplicate records are inadvertently created. When a potential duplicate record is created, but you do not have access to the record to confirm if it is indeed a duplicate, contact the Facility associated with the potential duplicate. This information can be located in the Primary Facility column in the National Waitlist, the Living Donor Program column in the Living Donor List, the ODO in the Deceased Donor List and the Transplant Centre in the Recipient List.

Once the facility is reached, indicate to them that you may have discovered a potential duplicate record and work together to determine if the two records are indeed duplicates. Once determined, corrective action can be taken; if the record is a duplicate, determine which record is the current functional record and flag the other record as a duplicate. If the record is not a duplicate, consider adding an Alternate ID to further distinguish them from each other.
