My patient has moved, now what do I do?

When a patient moves, their record in the Registry remains valid, but three changes are required:

  •  A change in the patient’s contact information and address
  • A change in the patient’s health card number/province
  • A change in the patient’s medical team
  • A change in the patient’s facilities

The first three changes can be done in the Registry with no difficulties.

  • A change in the patient’s contact information and address.
    • Open the patient’s record
    • Click on the Contact tab
    • Scroll down to the Contacts section
    • Add/delete or modify existing contact information
    • Click the Update button
    • Click the Save icon
    • Scroll down to the Address section
    • Add/delete or modify existing address information
    • Click the Update button
    • Click the Save icon
  • A change in the patient’s health card number/province
    • Open the patient’s record
    • Click on the Personal tab
    • Scroll down to the Principal IDs section
    • Change PHN/Home Province
    • Change PHN number
    • Click the Save icon
  • A change in the patient’s medical team
    • Open the patient’s record
    • Click on the Contact tab
    • Scroll down to the Physicians’ section
    • Add/delete or modify the existing physician name, type and phone number
    • Click the Update button
    • Click the Save icon

Changing the patient’s facilities should be the last step performed, as once the change is made, you will lose access to the patient’s record. This is best coordinated with the new transplant center by telephone, with a clear explanation of what will be done (a change to the patient’s facility to the new facility) along with the patient’s CTR ID number

  • A change in the patient’s facilities
    • Open the patient’s record
    • Click on the Contact tab
    • Locate the Facilities section
    • Click the Add button
    • Select the facility type
    • Select the facility
    • Click the Update button
    • Repeat to add all required new facilities
    • Click the Save icon
    • Confirm by telephone that new facilities have access to patient record
    • Return to patient record
    • Click on Contact tab
    • Locate the Facilities section
    • Delete original facilities from list
    • Click the Save icon
